As an <a href=\"https:\/\/www.hhs.gov\/cto\/initiatives\/open-innovation\/about\/index.html\" target=\"_blank\" rel=\"noreferrer noopener\">open innovation<\/a> manager, his mandate was as abstract as it was critical: to help the agency use new tools and strategies to find innovative solutions to serious health challenges.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">Today, Patel is a leading federal innovator who has elevated kidney disease as a key national health priority. In 2017, he launched <a href=\"https:\/\/www.kidneyx.org\/\" target=\"_blank\" rel=\"noreferrer noopener\">KidneyX<\/a>, a multimillion dollar public-private partnership between HHS and the American Society of Nephrology that accelerates new ways to treat, diagnose and prevent kidney disease. Two years later, he helped draft an executive order\u2014the <a href=\"https:\/\/www.asn-online.org\/policy\/webdocs\/page.aspx?code=220\" target=\"_blank\" rel=\"noreferrer noopener\">Advancing American Kidney Health Initiative<\/a>\u2014that requires HHS to meet ambitious benchmarks for improving nationwide kidney health.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">This monumental shift would not have occurred without Patel\u2019s ability to lead change. He became interested in the federal grant-making process as a doctoral student in physical chemistry and later witnessed how certain health processes and interventions could shape people\u2019s lives as co-founder of OmusonoLabs, a Uganda-based company working to empower local residents to design and implement new 3D printing services.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">Patel said both experiences led him to become \u201cacutely aware of the transition from science to societal impact\u201d and motivated him to work at HHS.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">\u201cThe department had its hands on the levers of all the things I was interested in,\u201d he said.&nbsp;&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">Still, Patel knew little about government, the agency or his specific job when he started as an open innovation manager at HHS. He recalled struggling initially to \u201cwrap my brain around who does what\u201d in the department and admitted to having only a vague sense of how to drive open innovation\u2014a strategy organizations use to tap new ideas and resources from external partners to solve problems.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">\u201cI couldn&#8217;t tell you what that meant when I applied for the job,\u201d he said. \u201cI did my due diligence, but it was still a slightly foreign concept,\u201d he said.&nbsp;<\/p>\n\n\n\n<div style=\"height:25px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n<\/div>\n\n\n\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:33.33%\">\n<div style=\"background-color:#f3f3f4\" data-bg-color=\"#f3f3f4\" data-bg-color-opacity=\"100\" data-border-thickness=\"0\" class=\"wp-block-pps-gutenberg-blocks-styled-container pps-styled-container px-4\">\n<p style=\"font-size:15px\"><strong>From the story: <\/strong>What challenges do you see in your organization, field or agency that people think are too big to tackle? Instead, people tended to work on more concrete projects within specific teams and silos\u2014a behavior often rewarded in government, according to Patel.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">\u201cWhat I noticed is that people get blinders on very quickly. You have your thing in front of you and you just do it. People are very stuck on the way of doing things\u2014unintentionally most of the time.\u201d&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">Patel overcame this hesitancy by focusing on the problems people were trying to solve, rather than promoting open innovation by name. He also established trust and rapport with people across the department through frequent\u2014and often impromptu\u2014conversations.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">Designed as free-flowing learning sessions without a set agenda, the discussions helped Patel gather information about the agency\u2019s health priorities and enabled him to demonstrate how open innovation tools\u2014prize competitions, crowdsourcing, new types of partnerships and more\u2014could serve employee needs. Working within the Office of the Secretary also gave Patel the credibility and \u201cair cover\u201d to make staff feel more comfortable taking risks.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">\u201cI usually started with a big problem and let people talk about what they do. It became more of a dialogue instead of me coming in with an agenda, which would have made people get tense and react.\u201d&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">Patel said the strategy made people more receptive to the concept of open innovation and helped him learn about the agency\u2019s work more broadly. In turn, he became a \u201cconnector,\u201d someone who could spark innovation by pointing people toward useful resources and potential partners.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">\u201cOne of the biggest barriers [to innovation] is that people need to be inspired. And the easiest way to inspire people is to connect them with someone who can help inspire them,\u201d he said.&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">Patel also inspired others by sharing his own thoughts about a given problem\u2014even if he did not have a perfectly formed solution first. He said the approach has encouraged agencies to lean into risks and institutionalize more experimental ways of addressing health issues.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">For example, Patel helped the Maternal and Child Health Bureau launch a <a href=\"https:\/\/mchbgrandchallenges.hrsa.gov\/\" target=\"_blank\" rel=\"noreferrer noopener\">challenge program<\/a> that has focused on promoting remote pregnancy monitoring and better access to healthy food for low-income families. He also supported the development <a href=\"https:\/\/innovation.cms.gov\/innovation-models\/artificial-intelligence-health-outcomes-challenge\" target=\"_blank\" rel=\"noreferrer noopener\">of a new initiative<\/a> at the Centers for Medicare and Medicaid designed to illustrate how artificial intelligence can be used to predict health outcomes for Medicare beneficiaries.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">\u201cPeople tend to keep things close to themselves because they want to get it perfect before they bring it into the world. I like being hyper-transparent about everything that I&#8217;m thinking about.&nbsp; I sort of take the risk for people and they come follow me.\u201d&nbsp;&nbsp;<\/p>\n<\/div>\n\n\n\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:33.33%\">\n<div style=\"height:750px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<div style=\"background-color:#f3f3f4\" data-bg-color=\"#f3f3f4\" data-bg-color-opacity=\"100\" data-border-thickness=\"0\" class=\"wp-block-pps-gutenberg-blocks-styled-container pps-styled-container px-4\">\n<p style=\"font-size:15px\"><strong>From the story: <\/strong>What tactics did Patel use to influence people? He was surprised by what he learned: Millions of Americans are on dialysis every week with no access to a transplant\u2014hooked up to a machine for hours while experiencing lasting side effects.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">He also learned that dialysis is costly. Recent reports show that <a href=\"https:\/\/www.cms.gov\/newsroom\/press-releases\/cms-announces-transformative-new-model-care-medicare-beneficiaries-chronic-kidney-disease\" target=\"_blank\" rel=\"noreferrer noopener\">about 20%<\/a> of Medicare funds are spent to treat those suffering from kidney disease and that patients on dialysis have an average life expectancy of <a href=\"https:\/\/www.kidney.org\/atoz\/content\/dialysisinfo#:~:text=Average%20life%20expectancy%20on%20dialysis,20%20or%20even%2030%20years.\" target=\"_blank\" rel=\"noreferrer noopener\">just five to 10 years<\/a>.&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">When Patel talked to these patients, he decided it was time to act. \u201cYou talk to enough dialysis patients and you&#8217;re like, \u2018This is crazy. How are we letting this persist?\u2019\u201d&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">In turn, he met with people across government and the private sector to discuss better alternatives to dialysis\u2014even tracking down the person who wrote dialysis policies for the Centers for Medicare and Medicaid Services: Abigail Ryan, <a href=\"https:\/\/www.cms.gov\/About-CMS\/Agency-Information\/CMSLeadership\/Office_CM\" target=\"_blank\" rel=\"noreferrer noopener\">currently<\/a> CMS\u2019 deputy director of the Division of Chronic Care Management.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">\u201cAs soon as I talked to her, something clicked. She got exactly what we were trying to do. She was doing her own thing, but we connected with her. She felt inspired and saw what was possible,\u201d he said.&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">Still, getting others to buy in to the need for action proved challenging. Some believed the government had already invested enough in kidney health research. Others believed that scientists lacked the technology to enable a real breakthrough. And several more found alternative approaches to reducing kidney disease too risky and unproven.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">He also realized health experts in government tended to work on isolated aspects of kidney health that made the issue seem overwhelming and impossible to tackle.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">\u201cIt was a third rail issue,\u201d he said. \u201cNobody wanted to touch it.\u201d&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">To overcome this skepticism, Patel knew he had to create a platform that brought people together to develop \u201cholistic\u201d solutions to kidney health\u2014including researchers and innovators from the private sector who were developing new solutions to kidney disease.&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">A core group of federal health leaders championed this vision and worked to make it a reality, including Jarah Meador, former Innovation Sourcing Lead at the Department of Veterans Affairs and a <a href=\"https:\/\/www.challenge.gov\/blog\/july-31-2021-why-should-government-leaders-invest-in-open-innovation\/\" target=\"_blank\" rel=\"noreferrer noopener\">fierce advocate<\/a> of open innovation.&nbsp;&nbsp;&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">Nevertheless, he recognized that some in government would take the new initiative as a sign of criticism that they had failed at their job. As a result, he was careful to pitch the initiative as something that capitalized on work already being done.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">\u201cIt happens a lot in government where anytime you propose something new, it\u2019s seen as a zero-sum game,\u201d he said. \u201cWe tried to frame this more positively by saying \u2018Everybody\u2019s doing what they\u2019re supposed to be doing\u2014we just need to accelerate your efforts.\u2019\u201d&nbsp;&nbsp;<\/p>\n<\/div>\n\n\n\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:33.33%\">\n<figure class=\"wp-block-embed is-type-video is-provider-vimeo wp-block-embed-vimeo wp-embed-aspect-16-9 wp-has-aspect-ratio\"><div class=\"wp-block-embed__wrapper\">\n<iframe loading=\"lazy\" title=\"Sandeep Patel Vision\" src=\"https:\/\/player.vimeo.com\/video\/647809459?dnt=1&amp;app_id=122963\" width=\"500\" height=\"281\" frameborder=\"0\" allow=\"autoplay; fullscreen; picture-in-picture; clipboard-write\"><\/iframe>\n<\/div><\/figure>\n\n\n\n<div style=\"height:350px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<div style=\"background-color:#f3f3f4\" data-bg-color=\"#f3f3f4\" data-bg-color-opacity=\"100\" data-border-thickness=\"0\" class=\"wp-block-pps-gutenberg-blocks-styled-container pps-styled-container px-4\">\n<p style=\"font-size:15px\"><strong>From the story: <\/strong>How did Patel try to ensure that others wouldn\u2019t view his program as a criticism of their own work? In 2017, Patel launched an ambitious public-private private partnership\u2014the Kidney Innovation Accelerator, or KidneyX\u2014that redefined how the federal government tackles kidney disease.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">The initiative began as a \u201cmoonshot approach to developing alternatives to dialysis.\u201d According to Patel, it\u2019s goal was to provide a platform for groups that rarely worked together\u2014both within and outside government\u2014to solicit, design and implement new innovations in kidney health.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">It took little time for KidneyX to dispel lingering questions about whether finding alternatives to dialysis was possible.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">Attracted by funding and networking opportunities, kidney health researchers from the academic and private sectors flocked to the initiative. They attended summits, participated in listening sessions with KidneyX leadership and submitted their work to prize competitions designed to incentivize the creation of improved dialysis products, more patient-driven kidney health solutions and the world\u2019s first artificial kidney.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">Realizing that these interventions composed just \u201cone piece of a larger problem,\u201d the initiative has also expanded its focus to building a more efficient organ transplant system.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">\u201cWe created this space where everybody came to us\u2014we had investors all over the country and companies coming in. People saw the issue as exciting.\u201d he said.&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">The new program also led to a shift in national policy. Just two years after KidneyX launched, President Trump signed an <a href=\"https:\/\/www.federalregister.gov\/documents\/2019\/07\/15\/2019-15159\/advancing-american-kidney-health\" target=\"_blank\" rel=\"noreferrer noopener\">executive order<\/a> that established the Advancing American Kidney Health initiative.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">The order\u2014the only in U.S. history that focuses specifically on a disease\u2014called upon HHS to achieve goals that align with the KidneyX mission: preventing kidney failure through better diagnosis and treatment, encouraging the development of an artificial kidney, and creating Medicare payment options that help patients better prevent kidney disease and incentivize kidney transplants for those on dialysis.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">Five months later, Trump signed a spending bill that included $5 million in funding for KidneyX\u2014the first time Congress supported prize competitions in kidney health and a sign that national policymakers had taken note of the initiative\u2019s innovative work. Feature stories <a href=\"https:\/\/www.youtube.com\/watch?v=yw_nqzVfxFQ\" target=\"_blank\" rel=\"noreferrer noopener\">on dialysis<\/a> and <a href=\"https:\/\/www.tbs.com\/shows\/full-frontal-with-samantha-bee\/clips\/organ-donation-an-american-horror-story\" target=\"_blank\" rel=\"noreferrer noopener\">organ transplants<\/a> on hit shows like \u201cLate Night with John Oliver\u201d and \u201cFull Frontal with Samantha Bee\u201d also signaled growing popular interest in the subject.&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">For Patel, the catalytic effect of KidneyX illustrates the benefits of taking risks and embracing uncertainty. While the initiative started as an untested public-private partnership with a broad goal, it nevertheless provided key players with the platform they needed to collaborate and experiment with new ideas in kidney health.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">In the last two years alone, these ideas have included the development of a <a href=\"https:\/\/techcrunch.com\/2020\/08\/09\/venostent-has-a-new-technology-to-improve-outcomes-for-dialysis-patients\/\" target=\"_blank\" rel=\"noreferrer noopener\">new bioabsorbable wrap<\/a> that supports cell growth after dialysis access surgery, <a href=\"https:\/\/www.healio.com\/news\/nephrology\/20190712\/telehealth-device-to-detect-thrombosis-among-winners-of-kidneyx-competition\" target=\"_blank\" rel=\"noreferrer noopener\">a telehealth device<\/a> that detects blood clots for patients on dialysis and <a href=\"https:\/\/abc7news.com\/ucsf-artificial-kidney-project-dialysis-transplants-university-of-california-san-francisco\/6346001\/\" target=\"_blank\" rel=\"noreferrer noopener\">a small internal filtration device<\/a> for the body that performs all the actions of a dialysis machine\u2014a critical technological component of an artificial kidney.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">Another KidneyX prize winner has also <a href=\"https:\/\/www.kidneyx.org\/PrizeCompetitions\/PastCompetitions\/redesigndialysisphaseiproposal17\" target=\"_blank\" rel=\"noreferrer noopener\">helped make<\/a> regenerative kidneys a viable health option for patients suffering from chronic renal failure.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">\u201cThat was the lesson\u2014don\u2019t make [investors and stakeholders] do the work. One aims to develop vaccines and therapeutics <a href=\"https:\/\/drive.hhs.gov\/beyondtheneedle.html\" target=\"_blank\" rel=\"noreferrer noopener\">without using needles and syringes<\/a>; others hope to create better <a href=\"https:\/\/drive.hhs.gov\/homediagnostics.html\" target=\"_blank\" rel=\"noreferrer noopener\">at-home testing services<\/a> and digital health tools like <a href=\"https:\/\/drive.hhs.gov\/digitalmcm.html\" target=\"_blank\" rel=\"noreferrer noopener\">smartphone apps and web platforms<\/a>; and still another is working to create <a href=\"https:\/\/drive.hhs.gov\/immunechip.html\" target=\"_blank\" rel=\"noreferrer noopener\">tissue chips<\/a>\u2014small 3D chips designed to show how human cells respond to drugs and other variants\u2014that model components of the human immune system.&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">Recently, three BARDA leaders oversaw the development of a vaccine, two therapeutics and a diagnostic test designed to stop the spread of Ebola in Africa. The leaders were <a href=\"https:\/\/servicetoamericamedals.org\/honorees\/boucher-lee-wolfe\/\" target=\"_blank\" rel=\"noreferrer noopener\">recognized<\/a> as <a href=\"https:\/\/servicetoamericamedals.org\/\" target=\"_blank\" rel=\"noreferrer noopener\">2021 Samuel J. Heyman Service to America Medal<\/a>\u00ae finalists for these accomplishments.&nbsp;&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">Patel believes the work\u2014like his work at KidneyX\u2014provides people with better service and a more favorable impression of government than they had before.&nbsp;&nbsp;&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">\u201cPeople&#8217;s impressions of government are acute\u2014an accumulation of little interactions that we have with people. We try to leave people with a good impression of who we are.\u201d&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">He said that none of this would be possible had he and his colleagues not been willing to innovate, take risks and develop a vision for change.&nbsp;<\/p>\n\n\n\n<p style=\"font-size:18px\">\u201cI think the general idea is that you take a lot of shots on goal and something eventually works and makes an impact.
